Kosney’s Death/Kosney’s Lair
I give up! I have been through MMR a dozen times and tried all 8 mirrors in the top right room and all the associated teleports but still unable to locate Koszey’s Lair. See attached map

You've already found the entrance. Look at the lower part of your map with the levers that opened a gate and then there is another gate right next to it. If you go through those two gates and look south, you should see a ladder. That ladder takes you to Kozney's Lair.
The ladder is indicated by a black line in the center of the far south of your picture, right after all those nice levers and the two gates.
