Crystal skull location

Please, can anyone tell me the locations of the crystal skull. I found four. I think there are five of them.

    One is on a shelf near Hazarex in Cave of the Prisoner. One on a shelf in Crypt of the Fallen. There is one in each undead dungeon. Hazarex can only enhance that weapon four times so if you have a fifth, it won't help you.

    Thx for HELP. I found all 5 Crystal skulls in the following locations:

    • two in Cave of the Prisoner (on a shelf near Hazarex and one on a shelf in Throne Room of the King)
    • one on floor in Undead can Dance (Cygnusium)
    • one on amphora in Dream Never Dies (Ruins of Ancients)
    • one on a shelf in Crypt of the Fallen (i missed this Crystal skull)
